- A referral letter with a current list of medical co-morbidities, medications and allergies
- Recent radiographs:
- Hip – pelvis (Antero-Posterior) + hip (Antero-Posterior + lateral) views
- Knee – weight-bearing Antero-Posterior, Rosenberg (posteroanterior 45° flexion), lateral + skyline views
